Я писала так: здрасьте сир/мадам, пишет вам Женя, номер ЕОИ такой-то, вот доки на ИТА, среди них вы найдете, далее список, иногда с пояснениями, если там копия или оригинал. Все. Следующий абзац: чтоб вам лучше было во все воткнуться, имеются следующие непонятки и второй список по пунктам: в доке таком-то указано то-то, но на самом деле это то-то, муж в армии не служил, военника не будет, фамилию меняли 25 раз потому-то и т.д. И последний абзац: если какие вопросы возникнут, всегда рады, если дополнительно что-то надо - велкам, спрашивайте. И в конце синсерели йорз. В общем, в формате АЙЛТС.
у меня на 7 листов ковер леттер был), не считая statutory declarations (официальная и вольная версия)
все как полагается, первый лист с таблицей описывающей приложенные доки, телефоны и стандартные фразы приветствий и сопутствующих пожеланий, плюс семь листов. подредактирую на предмет конфиденциальности и выложу если надо.

Было содрано с emigratenz, уместилось на одном листе, похоже на Jane's. Если есть какие-то замечания, можно написать здесь (как я объяснил, что такое трудовая книжка
или в конце. Вообще, формы нет, кто как хочет, так и пишет.У меня получилось так:

A list of the documents enclosed is given below.
• Completed application form, together with payment details and photographs;
• Completed additional detail forms (my and wife’s);
• Certified copies and translations of full birth certificates (my, wife’s, daughter’s, and son’s);
• Certified copies of current passports (my, wife’s, daughter’s, and son’s);
• Certified copies and translations of my and wife’s National Identification Card as evidence of our relationship and living together (please find current registration address and a marriage record);
• A certified copy and translation of our marriage certificate;
• A reference letter from the condominium where we have a flat as proof of shared residence;
• Three photographs of me and my partner together.
• Reference letters from my previous employers confirming my experience.
• Reference letter together with pay slip from my current employer.
• My Work Record Card and translation with dates of employment and occupied positions confirming my work experience. It is an official document in Russia containing records of person’s employment history, which must be filled by each HR departments when an employee starts and finishes working for a company. It is kept at the HR department and I got a copy certified by the head of our HR department;
• Certified copies and translations of my qualifications: academic testamur and transcripts;
• Reference letter from the University as an evidence of renaming;
• IELTS Test Report Form with photocopy;
• Certified copies and translations of my partner’s qualification and course transcripts together with translations. She has two qualifications, both are included;
• Police certificates (my and wife’s);
• Medical and X-ray certificates (my, wife’s, son’s, and daughter’s);
• Migrant Levy payment form.
